Lovelace Family Medicine is accepting new patients!
If you are interested in becoming a new patient the first thing we will need is a copy of your previous medical records. There are two easy ways to do this. The first is to ask your previous doctor’s office to send us a copy of your records. You may also print a copy of our medical records request form located in the link at the bottom of this page and mail, fax, or drop this form off at our office and we will request a copy of your records from your previous doctor for you.
Once we have received a copy of your records, a provider of your choice at Lovelace Family Medicine will personally review them. After your records have been reviewed, your provider will notify the front desk to contact you to schedule your first appointment with the knowledge that your provider is already familiar with you, your particular medical needs, and what type of appointment is best for you.
When you arrive for your first appointment you will be asked to fill out our new patient paper work. If you would prefer to fill this out ahead of time you are more than welcome to print the forms from the following links and bring them with you to our office on the day of your visit.

STEP 2: Once you have been accepted as a new patient, complete the 1st appointment packet.
