Sara Ballentine
Born and raised in Chapin, S.C., Sara received her undergraduate degree in Psychology and Masters in Health Administration both from the University of South Carolina in 2014. She started with Lovelace Family Medicine working on quality improvement activities to eventually become the Practice Administrator.
Sara enjoys listening to podcasts, watching SNL, attending concerts, gardening, and spending time with her husband, Brad, child, Gavin, and dog, Gilly.

What is a Patient Centered Medical Home?
A Patient-Centered Medical Home (PCMH) is a model of primary care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety.
