Fees and Insurance

We will be happy to explain our fee schedule at any time.  Please ask to speak to someone in our business office if you have any questions about fees. Payment is expected at the time of service unless prior arrangements have been made.  For extensive services, we can arrange a monthly payment plan or other option if necessary.We contract services and will file your insurance for the following insurance carriers:

Please note that we do not accept any Medicare Advantage plans.
Please check with our front desk for any additions or deletions to this list.  You are always responsible for payment of non-covered services, co-payments, or deductibles at the time service is rendered.
