Appointments
Current patients are welcome to call (803)364-4852 at any time to set up an appointment. Lovelace Family Medicine is proud to offer same day appointments with each of our providers in order to accommodate our patient’s busy schedules and to still allow them to see the provider of their choice. As you know, you may be asked to fill out some paperwork upon arrival to our office. In order to make your appointment go as smoothly as possible we have included those forms in the links below. If you are able to, we ask that you please print these forms out and fill them out ahead of time to hand to our receptionist upon your arrival at the office.

Referrals
In the event your provider feels it is necessary refer you to a specialist we ask that you give our office 7-10 business days to set up your appointment. If you have not heard from us or the specialist’s office after that time please call our office to check on your referral. Tricare and Bluechoice patients, please be aware that if you plan to continue to see a specialist you may need to extend your referral beyond the original time-frame. If this is the case please call our office in advance of your next appointment date so that we will have time to request an extension of your authorization.
Release of Medical Information
In order to keep your medical information confidential, medical records will only be released once our office has received a signed authorization form. You can stop by the office and request a form to fill out or you may print the form at the link below and mail, fax, or drop the form off at the front desk.
